Help!!! 2 doses of methotrexate

Post by Victoria V_17 »

Hey everyone!!! Hoping to find some answers from you guys on my situation!!!! I received my 2nd dose of methotrexate in Tuesday because my levels weren’t going down from the week before. On Monday they were at 11,000 but on Tuesday right before the 2nd dose of shots they were at 7,782. I thought it was finally working and yesterday they checked again and now they’re saying it’s at 10,000. I’m soooo confused as to why this is happening! Has this happened to anyone and what was the end results! I’m not in any pain and I really don’t want to have the surgery !

Dear Victoria V_17,
I am so sorry to hear of your ectopic pregnancy and loss,
I am unsure of your exact timeline from your second injection but the hCG level often rises on the day four blood test because the action of Methotrexate is not instantaneous, so the cells will have continued to divide for two or three days after the injection was given, and some cells release more hCG when they start to disappear. Your doctors are looking to see a drop in your hCG value of at least 15% between days four and seven. If there has not been a 15% drop, this is when the doctors will consider a second dose of Methotrexate or surgery.
Although I do not wish to alarm you, if you experience any abnormal abdominal pain, bleeding, shoulder tip pain or feel dizzy and unwell please seek urgent medical advice.
Sending much love and positive thoughts,
Karen x

Thank you Karen! My doctor called me yesterday for my results again and said it had gone down so it looks like it’s working! Thank god! I was sooo worried and just wanted this to go well after dealing with so much emotionally and physically as well.

Dear Victoria V_17,
That's such good news.
I am afraid it is difficult to predict how long it will take for your hormone levels reach non-pregnant levels. As we are all individual, our bodies respond differently to the drug and it depends on factors like how high levels reached and our unique physiology. What I can say is that it can take a number of weeks and, while it can take some time, it is not as invasive a procedure as surgery. It is encouraging that your levels are now falling although I appreciate that it can be a drawn out process. Also, in terms of chances of future successful pregnancies, studies do not show a significant difference between treatment routes, whether surgical, medical using methotrexate or expectant management (allowing time for the body to resolve the pregnancy itself).
Sending much love and positive thoughts that they continue to reduce,
Karen x
