Fluid on second scan, Second ectopic? Failing or too early??

Post by Aqua_Maria »

Hi All,

I am currently pregnant as I have had positive pregnancy tests plus I have completely missed this months period so far. I had an ectopic last year so have been visiting EPU for bloods and internal scans. My first set of bloods were 1054 and the second lot (taken 48 hours later) were 1400 and something. I have had two internal scans the first one the lady said my uterus wall had thickened and there was a white shadow in there which could be implantation so a possible very early pregnancy this was last Friday and it was put on my notes as PUL thats when I was sent for bloods.

This Wednesday just gone I had a repeated scan and again nothing was seen no sign of ectopic or pregnancy lady said she thought it may be too early she asked if I had been bleeding which I hadn’t as she could see some fluid somewhere but never went into detail about where this was or the reason for it.

The Nurses at EPU wanted to get the doctors opinion so I received a phone call yesterday saying they wanted me in for another blood test this Friday to have a look at my levels as they said they weren’t high enough for it to be a viable pregnancy so suggested I had some pregnancy tissue which was making me “feel” pregnant but it was a failing pregnancy nonetheless this is there thoughts.

I then received a later call from the same Nurse at EPU who said the specialist consultant had taken a look at my notes and wanted to scan me himself as I am at risk for a second ectopic so I have no idea what the outcome of all this will be. I suppose I am writing for some advice and maybe some positive stories as I know what an ectopic mmc and mc all feel like as I have experienced all of these and quite recently.

Thank you for reading I know it is a long post.

Hoping for a rainbow baby so keeping my fingers tightly crossed.

Dear Aqua_Maria,
Whispered congratulations on your pregnancy,
If scans are inconclusive, doctors would carry out a series of blood tests to check hCG levels to see if the numbers are rising as expected. However, in isolation, hCG tests only provide a part of the picture - declining levels indicate that a pregnancy has ended and will be miscarried. It can also possibly indicate an ectopic pregnancy that is "self-resolving". Also hCG levels rising by less than 66% over 48 hours means it is likely (but not certain) that it is ectopic. If levels rise normally, they suggest the pregnancy is implanted in the uterus. You may see from this that hCG levels only indicate certain possibilities and are not in themselves a definitive guide. A positive identification of an implanted embryo on a scan is usually needed before a final diagnosis/certainty that the pregnancy has implanted in the uterus. This is possibly the reason why your doctors are waiting until your appointment to carry out scans and tests.

As you have mentioned that your scan didn't show an implanted embryo, this is known as a Pregnancy of Unknown Location or PUL. This is not a diagnosis but a label until the final location can be identified with certainty. I cannot be sure, but it could be the case that it may be too early to see a pregnancy on the scan.

Sending much love and positive thoughts,
Karen x
